NASW-PA Annual Awards Nomination Form

 

It is time to nominate colleagues for their outstanding efforts within the social work profession. Take a moment and look around at your immediate co-workers and other colleagues with who you have contact! See whom you can nominate for our 2019 awards. Awards will be presented at the Kalarhari Resorts in Mount Pocono during the NASW-PA / PASWE Annual Conference, September 22-25, 2019. Award presentations will take place Tuesday, September 24, 2019 from 6 - 9 pm (exact time may slightly fluctuate).

 

Social Work Educator of the Year

Candidates must:

1.       Have made a demonstrable difference in social work education within the past 3 years or the results of such efforts became apparent in the past year

2.       Demonstrate leadership

3.       Take risks to achieve results

4.       Be a member in good standing of the NASW-PA Chapter

 

  

Kristin Federici Bowser  - Young Social Worker of the Year

Candidates must:

1.       Have a history of outstanding service in the following area(s);

a.       Advocacy for clients

b.       Impact on social policy

c.       Exceptional practice

d.       Program creation

2.       Be under 30 years of age

3.       Demonstrate professional leadership

4.       Hold a social work degree

5.       Be a member in good standing of NASW-PA Chapter

 

 

Public Citizen of the Year

Candidates must:

1.       Make a significant contribution to an area or population of concern to social work such as:

a.       Public social issues

b.       Populations at risk

c.       Quality of life for communities

2.       Produce work of lasting impact

3.       Act with courage and verve

4.       Demonstrate leadership

5.       Exemplify social work values and ethics

6.       Not hold a social work degree

 

 

Social Worker of the Year

Candidates must:

1.       Have made a demonstrable difference within the past 3 years, in an area such as those listed below or the results of such an efforts become apparent in the past year

a.       Advocacy for clients

b.       Impact on social policy

c.       Exceptional practice

d.       Program creation

e.       Innovative research

2.       Demonstrate leadership

3.       Take risks to achieve results

4.       Be a member in good standing of the NASW-PA Chapter

 

 

Lifetime Achievement

Candidates must:

1.       Demonstrate repeated achievement throughout their career as a social worker.

2.       Receive recognition beyond the social work profession

3.       Make contributions with a lasting impact

4.       Have been in social work practice for a minimum of 15 years.

5.       Be a member in good standing of NASW-PA

 

Nomination Procedure for all awards:

1.       Provide the following information for the nominee and the nominator:

a.       Name

b.       Title

c.       Employer

d.       Email

e.       Phone number

2.       Nominator to provide a one page summary of demonstrated contributions (for continuity, please address these questions)

a.       What did the candidate do?

b.       What were the consequences?

c.       Who were the beneficiaries?

d.       What was accomplished?

e.       How do the accomplishments relate to social work (public citizen award only)

f.        What is the long term impact (lifetime achievement award only)

3.       Optional information

a.       Resume / Bio of nominee

b.       Supporting documents (ie letters of recommendations, media articles)

c.       Current photo of nominee

 

Nominations are due August 16, 2019
